| Wow  that's an headache!
Did you checked with another online game (best would be an FPS game) to see if the problem concerns only ET or any MMO/Online game?.
-If only ET-
As you know, with your FPS monitor and lagometer you can determinate if it's an FPS drop OR bandwidth issue.
If it's about network:
-(stupid question) Did you off/on your modem (or router) since you experience this problem? Often it fix lag problems.
- if not only ET-
1] run Command prompt with Administrator privileges.
There how to do it on Windows Seven/Vista.
2] type next command: netsh winsock reset all
Then restart your computer.
It's reset TCP/IP protocol stack for your network card(s)

| i did some combination with com_maxfps and com_hunkmegs which fixed the problem for now  thanks for all the help btw!
Now im just going to make a lowgraph cfg and hope it still works
